Job summary

Wirral University Teaching Hospital NHS Foundation Trust is seeking a Band 6 Nurse Practitioner to join the Unscheduled Care Coordination Hub (UCCH) at Wirral. Lead delivery of an innovative urgent care coordination model and oversee complex referrals to connect patients with appropriate community-based services.

Qualifications

• First Level Nurse with current registration with NMC
• Degree in relevant subject
• Post graduate qualification in rapid response/community nursing/emergency department/acute medical or equivalent experience
• Evidence of recent Professional Development
• Teaching and assessing qualification or equivalent
• Demonstrable post registration experience
• Non‑medical Prescribing (V300)

Responsibilities

• Prevent unnecessary hospital admissions by developing an effective interface between agencies and services.
• Manage patients with unplanned care needs, including hospital admission where appropriate.
• Provide advice and support to patients and carers and arrange timely referrals to relevant services.
• Ensure patients are referred to the most appropriate service or agency to meet their care needs.
