Job summary

Cheshire and Wirral Partnership NHS Foundation Trust is seeking an experienced Band 6 Nurse Coordinator to join our Single Point of Access Team in Ellesmere Port. The role involves triaging patients via NWAS and CAS 111 and coordinating community pathways to support hospital avoidance wherever possible.

You will also facilitate GP referrals to ensure patients receive the right care at the right time and place, across the West Cheshire footprint.

Qualifications

• Band 6 Nurse Coordinator experience preferred.
• Experience in triage and care coordination is desirable.
