Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is seeking Community Nurses to cover the Fylde Coast, with permanent appointments across clinics and patient homes. The role requires NMC Level 1 Registration (Adult) and Enhanced DBS clearance, and a valid car driver’s license.

An exciting opportunity has arisen for the recruitment of Community Nurses based across the Fylde Coast covering clinics . The advertised roles are permanent positions.

The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients’ homes or clinic settings.

To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.

The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.

To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.
