Job summary

Routes Healthcare in Chesterfield is seeking a Nurse Case Manager to oversee complex care packages and ensure high-quality, person-centred care. You will lead care planning, risk assessments and clinical governance while coordinating with a multi-disciplinary team.

The role requires an RN qualification and an active NMC pin, with preference for those experienced in high dependency care. A car is required, and the position offers a full-time, permanent schedule with some evenings and weekend

Qualifications

  • RN qualification required.
  • A valid NMC pin.
  • Experience in high depending or critical care is advantageous.
  • Tracheostomy and ventilation management in community setting desirable.
  • Computer literate with Word, Excel and Outlook.
  • Access to a car for work.

Responsibilities

  • Management of complex care packages, clinically and holistically.
  • Develop, review and implement care plans and risk assessments.
  • Deliver training to the care team and maintain appropriate skill mix.
  • Engage in networking, MDT meetings and work with clinical commissioning groups.
  • Carry out competency assessments with the care staff team.
  • Audit client notes and medication records.

Salary:Up to £45,000 depending on experience

Contract:Full-Time, Permanent Monday/Friday with some ad hoc flexibility for evenings and weekends

Your typical day-to-day will include:
  • Management of complex care packages, both clinically and holistically
  • Developing, reviewing and implementing care plans and risk assessments
  • Manage and deliver the training need of the care team, maintaining an appropriate skill mix to ensure safe and effective clinical practice
  • Engaging in networking, attending MDT meetings and work with clinical commissioning groups
  • New and ongoing competency assessments with the care staff team
  • Auditing of client notes and medication records
What we're looking for from you:
  • RN Qualification
  • A valid NMC pin
  • Experience in a high dependency or critical care setting would be advantageous
  • Experience in tracheostomy and ventilation management in a community setting is desirable
  • Be computer literate and have experience of using Word, Excel and Outlook
  • Someone passionate about putting people first
  • Flexible and autonomous
  • Organisational & time management skills
  • An understanding of CQC regulations
  • Excellent communication skills, both written and verbal
