Job summary

Routes Healthcare is seeking a dedicated Care Coordinator to schedule and coordinate home care visits, ensuring our clients receive outstanding support in Preston and surrounding areas. You will manage care runs, adapt to changes in staff availability, oversee scheduling through our system, and coordinate with the Registered Manager to deliver high-quality, compliant care in line with CQC requirements.

A full UK driving licence is essential.

Qualifications

  • Organisational skills and time management essential.
  • Excellent communication with clients, families and teams.
  • Driving licence required to travel to client sites.
  • Understanding of regulatory standards (CQC) desirable.

Responsibilities

  • Coordinate delivery of home care visits using scheduling system.
  • Adapt to changes in staff availability and client needs.
  • Ensure staff attend meetings, supervision and training within required timescales.
  • Assist Registered Manager to deliver high-quality care service.
  • Manage on-call duties on a rotating basis.

  • Someone passionate about putting people first
  • Organisational & time management skills
  • An understanding of CQC regulations
  • Superb communication skills, with the ability to coordinate a team of healthcare workers
  • An NVQ in Health and Social Care would be a big bonus
  • Due to the nature of the role, you will need to have a full valid UK driving licence with access to your own vehicle
Your Typical Day To Day Will Include
  • Co-ordinating the delivery of care using our care scheduling system
  • Adapting to changes in schedules, including staff availability and new clients into existing care runs
  • Ensuring that healthcare workers attend staff meetings, supervisions, and training within CQC-required timescales.
  • Assisting your Registered Manager to deliver a high-quality care service
  • Manage on-call duties on a rotating basis
