Job summary

Routes Healthcare in Yorkshire seeks a Registered Nurse to join as Nurse Case Manager, overseeing complex care packages and driving clinical excellence across our Yorkshire services.

You will develop and review care plans, lead training for the care team, attend MDT meetings, and contribute to high-quality outcomes while adhering to CQC requirements.

Qualifications

• Registered Nurse with a valid NMC pin.
• Experience in high dependency or critical care.
• Tracheostomy and ventilation experience (community setting desirable).
• Strong communication, organisation, and IT skills.
• Understanding of CQC regulations.
• Car owner/driver.
