Job summary

Sirona care & health in Bristol, North Somerset or South Gloucestershire is seeking a Nurse Associate or Assistant Practitioner to join a multi-disciplinary team and make a real difference in the community.

You will work 8am–8pm on a rota across seven days, provide patient‑centred care, use electronic records, and support learning for trainees while upholding NMC standards and maintaining a safe environment.

Qualifications

  • Higher Apprenticeship in Health level 5 or Foundation Degree in Health and Social Care.
  • Current NMC registration (if Nursing Associate).
  • Willing to undertake any training relevant to the role.

Responsibilities

  • Undertake holistic assessments and implement care programmes for patients who are acutely or chronically unwell.
  • Assess and monitor service users' health needs and wellbeing.
  • Use clinical reasoning to assess, plan, implement and evaluate patient-centred interventions.
  • Collaborate with service users, carers and family to identify goals.
  • Contribute to appropriate referrals to other services.
  • Work within a multi-disciplinary team to provide integrated care and share knowledge.
  • Assess and arrange provision of equipment.
  • Support and educate service users and carers to promote independence and safety.

Are you an enthusiastic and passionate Nurse Associate or Assistant Practitioner looking for a fresh start? Come and join Sirona in a role where you can make a real difference to the community you live in.

You will work in a team with other multi-disciplinary professionals including Nurses, Physiotherapists, Occupational Therapists, Administrators and other support staff, on a rota system between the hours of 8am-8pm over a 7 day working period.

You will need excellent communication and clinical skills and have experience of working within a team. You will be required to use electronic patient record systems, in addition to this you will have good time management and be able to prioritise your own work load.

A full driving licence and access to a vehicle is required for this role and we have full time and part time positions available!

Main duties of the job

Undertake appropriate delegated initial holistic assessments and implement programmes of care for patients who are acutely and chronically unwell

Assess and monitor service users' health needs, assessing patient condition and well-being

Use clinical reasoning skills, within the designated competency framework, to assess, plan, implement and evaluate, patient- centred intervention programmes

Collaborate with service users, carers and family in identifying appropriate and achievable goals

Contribute to the facilitation of appropriate referrals to other services

Work within a multi-disciplinary team, to provide integrated skills and share knowledge

Assess and arrange the provision of equipment

Take every opportunity to support and educate service users and their carers to promote independence, good health and personal safety

Provide and maintain a safe and comfortable environment for all patients and carers

Support, supervise, assess and act as a role model to other learners such as Nursing Associate trainees, first year student nurses and health care support workers as required

Maintain accurate, informative and legal records of all interventions in accordance with policy

Actively participate in training; this may involve assisting in the delivery of training

Take responsibility for your own delegated objectives, training, proficiencies and competence, as outlined by the NMC code of conduct

Support delivery of vaccination campaigns

  • Experience of working in a health or social care setting providing direct patient care
  • Ability to conduct clinical tasks within the Nursing Associate scope of practice to a high level of competence
  • Clear understanding of NMC standards of proficiencies and professional standards of practice including revalidation
  • Effective communication skills both written and verbal
  • Range of IT skills
  • Contribute to the holistic assessment of a service user
  • Ability to manipulate fine tools e.g. for venepuncture or other clinical tasks
