Job summary

Sirona care & health is seeking a compassionate Support Worker to join our MDT and deliver therapy-based support in community settings. You will work with patients in their homes, alongside nurses and therapists, to improve independence and functional abilities.

We offer enhanced weekend pay, generous annual leave, and a staff benefits scheme. Full and part-time hours are available with flexible patterns across 8am–8pm, 7 days a week.

  • NVQ Level 3 in Health & Social Care or willingness to work towards.
  • Experience of direct patient care in a health or social care setting.
  • Ability to use IT systems, including EMIS, to document patient progress.

  • Work closely with patients to deliver therapy interventions under the guidance of health care professionals.
  • Support patients in developing skills for daily living and independence.
  • Assist patients in setting realistic goals and maintaining motivation.
  • Monitor and record progress using IT systems and report to the MDT.

As a Support Worker, you will play a vital role within a multidisciplinary team (MDT) dedicated to helping individuals regain independence and achieve their rehabilitation goals.

Your work will focus on providing therapy-based support in community environments, empowering clients to improve their functional abilities and quality of life. As well as working along side the community nursing team to provide treatment for patients within their own homes.

We are seeking candidates who are compassionate and patient with a strong desire to support others. Good communication and interpersonal skills are essential, with the ability to work independently (including lone working) and as part of the wider MDT team.

A basic understanding of therapy principles, rehabilitation techniques and nursing skills and techniques will be required (training provided if needed). This includes Observations, Phlebotomy, Would Care and Administration of Insulin. Relevant experience in community settings is desirable.

Main duties of the job
  • Work closely with patients to deliver tailored therapy interventions under the guidance of qualified health care professionals, including Physiotherapists, Occupational Therapists and Registered Nurses.
  • Support patients in developing and practicing skills needed for daily living and independence.
  • Assist patients in setting realistic goals and motivate them to work towards achieving these objectives.
  • Monitor and record patients’ progress, using programmes such as EMIS, as well as providing feedback to the MDT and lead health care professional.
  • Promote a safe and supportive environment, encouraging patients’ confidence and participation.
  • Collaborate with other team members, including physiotherapists, occupational therapists, and nurses, to ensure holistic care.
  • Encourage and facilitate community engagement and participation for patients.

Enhanced pay for weekends and bank holidays, mileage is reimbursed whilst you carry out your duties at work. We also offer generous annual leave allowance and an attractive staff benefits scheme.

We have full and part time hours available and can support flexible working patterns - currently this service runs from 8am - 8pm - 7 days a week.

Qualifications

  • Able to work without direct supervision, as agreed within the team, to achieve the objectives of the post.
  • Able to prioritise tasks and make correct decisions within a set of clear guidelines.
  • Experience of direct patient care in a health or social care setting.
  • Ability to manipulate fine tools e.g. for venepuncture & observations
  • Experience of team working and an understanding of team dynamics including what factors make a team work well and what can go wrong and why.
  • Able to use a variety of IT systems e.g. email, EMIS.
  • Recent relevant experience within a community setting.
