Coventry and Warwickshire Partnership NHS Trust is seeking a capable nurse-led coordinator to support a 24-hour respite/enhanced care service. You will work with a multi-disciplinary team to assess, plan, implement and evaluate care plans and packages, and you will often lead on shifts with limited supervision.

Responsabilidades

• Assist the Nurse Manager in delivering a 24-hour nurse-led respite/enhanced support service.
• Collaborate with the multi-disciplinary team to assess, plan, implement and evaluate comprehensive care plans and packages.
• Lead and support the team to provide high quality care with minimal supervision.
• Adaptable and flexible approach due to changing needs of the SU/CYP and service delivery.
• Support people with learning disabilities, complex health needs and behaviours of concern; monitor health deterioration using NEWS2 and adhere to care plans and risk assessments.
• Work nights to support a 24-hour rota.
• Monitor and manage medication following trust policy and ensure good medication practices.
• Support staff supervision and lead shifts to promote timeliness and teamwork.
• Complete audits to maintain trust standards and policies.
• Hold a caseload to assess and review care plans and risk assessments, being the key contact for families.
