Royal Free London NHS Foundation Trust is seeking a Band 4 Patient Pathway Navigator in Neurology to support the patient journey from referral to discharge at the Hampstead site. The role sits within a team of Patient Navigators/Clinical Pathway Administrators and focuses on streamlined administrative processes around the patient’s needs.

The post is a 4-month fixed term contract and may be offered as a secondment where appropriate.

Qualifikationen

• Be able to work within a multi-disciplinary team
• Be proactive, enthusiastic and resilient
• Be able to support and reassure patients attending the Neurology service
• Experience of working with various staff groups and excellent communication & organisational skills

• Maintain service delivery and improvement within the Neurology department
• Coordinate patient journeys from referral to discharge
• Support medical and nursing colleagues within the department
