Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Metastatic Breast Cancer Support Worker to provide personalised care coordination and practical and emotional support for patients living with metastatic breast cancer. You will work within the Breast Care Team, collaborating with CNS, consultants and wider cancer services to identify needs and coordinate care across pathways.

Qualifications

• Experience in coordinating patient care and signposting to support services.
• Ability to work collaboratively with multidisciplinary teams and primary care colleagues.
• Strong organisational and communication skills with empathy for patients and families.

Responsabilités

• Act as a designated key worker for patients with metastatic breast cancer and a main point of contact for families.
• Coordinate referrals, appointments, investigations and reviews to ensure timely care.
• Undertake Holistic Needs Assessments and support personalised care planning.
• Signpost patients to relevant support services and ensure access to information and resources.
• Assist in MDT meetings and maintain accurate patient records and data for audits.
