Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is seeking a compassionate Metastatic Breast Cancer Support Worker to assist patients across their cancer journey. You will coordinate care, provide information, and act as the main contact for patients and families within a multidisciplinary team.

You will undertake holistic needs assessments, develop personalised care plans, signpost services, and support timely access to information, treatment and services.

Qualifications

• Compassionate, motivated and organised individual with a patient-centred approach.
• Able to work across multidisciplinary teams and act as a key point of contact for patients and families.
• Experience in coordinating care, information provision and support throughout cancer pathways.

Responsibilities

• Act as a dedicated key worker and first point of contact for patients and their families
• Work closely with the Breast Clinical Nurse Specialists, consultants, oncology teams, palliative care services and wider MDT
• Undertake Holistic Needs Assessments and assist patients in developing personalised care and support plans
• Support the delivery of personalised cancer care
• Coordinate follow-up appointments, investigations, referrals and patient reviews
• Signpost patients to relevant services
• Identify patients who may require additional support and elevate to the CNS
• Provide emotional support and practical guidance
• Maintain accurate and timely records
• Support data collection, audits and quality improvement
• Participate in MDT meetings
• Work collaboratively with administrative teams
• Promote health and wellbeing initiatives and support patients in accessing resources
