Job Description - Mental Health Support Coordinator (25142)

Location: Hampshire West, based in Lymington & Hythe, New Forest, with ad hoc support across Hampshire West and our community-based Enhanced Support Service.

Type: Permanent, full-time (37.5 hpw), rota working including some weekends and Bank Holidays.

Salary: £13.21 per hour (£25,838 per annum) plus benefits including Health Cash Plan.

About the Role

As our Mental Health Support Coordinator, you'll be at the core of supporting customers with mental health needs to develop skills for independent living. Your work will make a significant difference in their lives, helping them achieve their goals.

Typical Day
  • Creating support plans with customers at our supported accommodation, collaborating with colleagues and specialist roles.
  • Supporting mental health management and access to wellbeing services.
  • Assisting with daily activities like cooking, shopping, budgeting, maintaining tenancies, and transitioning to independent living.
  • Conducting risk assessments, support planning, goal setting, and reviews; ensuring health & safety and property maintenance.
  • Partnering with external agencies such as Social Care, Mental Health Teams, GPs, and community services.
  • Preparing rooms for new customers.

  • Experience in maintaining accurate customer records.
  • Ability to work independently, stay calm under pressure, and be resilient.
  • Proficiency with technology for support plans, risk assessments, communication, and collaboration.
  • A vehicle insured for business purposes, with mileage paid by us.

Our Team

You will join our passionate team led by Client Services Manager Karen, involved in recruitment, co-producing training, and supporting customers towards independent living. We enjoy a friendly, diverse environment with interests spanning from snacks to motorbikes and Star Wars.

Job Details
  • Supporting customers across Hampshire West, including occasional travel to Eastleigh and Winchester, with mileage paid for additional travel.
  • Operating 7 days a week, with shifts from 9am-7pm on weekdays and 10am-6pm on weekends, averaging 37.5 hours per week.
  • Flexible scheduling within the team to meet customer needs, with opportunities for shift swaps and rota adjustments.
  • Monthly in-person team meetings and regular virtual sessions for training and reflection.
  • Provision of a Surface Pro and Mobile Phone to support your work.
  • An Enhanced DBS check will be required, paid for by us.
