As part of the Community Mental Health Team, you will:
Complete strengths-based assessments and care planning.
Assess, commission and review care and support packages.
Work within current legislation, policy and statutory guidance.
Undertake planned interventions to promote independence and wellbeing.
Safeguard adults who may be at risk or vulnerable.
Work collaboratively with health professionals and partner agencies to deliver effective multi-agency support.
Empower individuals to have choice, control and a strong voice in decisions affecting their lives.
Contribute to maintaining high-quality services across the city.
A recognised Social Work qualification.
Registration with Social Care Wales or Social Work England.
At least 6 months' UK experience working as a registered Social Worker.
An Enhanced DBS on the Update Service (or be willing to obtain one).
Three years of satisfactory references.
A full UK driving licence and access to a vehicle.
