Bridgend Council is looking for anexperienced Adults Social Worker to join their East and West Integrated Network Teams on initial 3 month contracts. Full-time candidate required but happy to receive applications from those considering part time. The East is based at Glanrhyd Hospital, Bridgend.
You will be working as part of a well-established multi-disciplinary team supporting adults aged 18+ with frailty, long-term conditions, disabilities and often additional mental health or emotional wellbeing needs.
Adults community social work within an Integrated Network Team
Managing complex cases with a strengths-based, person-centred approach
Care coordination and multi-agency working
Regular visits required (driver essential)
Hybrid working - 2 days from home, 3 days on site
Qualified Social Worker withpost-qualifying adults experience
Registered with Social Care Wales
Confident working within an MDT environment
Strong assessment, care planning and care coordination skills
Able to work autonomously while contributing to a wider team
Up to £35.32 per hour, dependent on experience
