Job summary

Hoop Social Work is seeking an Social Worker to join the Community Mental Health Team in Caerphilly. The role is an immediate start on a 6-month contract with some home and office-based working and requires driving to visits across the borough.

In this post you will undertake assessment and care management, chair MDTs, complete duty rosters, and contribute to care coordination. Registration with Social Care Wales or Social Work England is essential.

IMMEDIATE START

2 Agency Social Workers are required to join the CMHTs in Caerphilly, opportunities available in both the North and South Teams (Rhymney and Caerphilly Town). This contract will be offered as an initial 6 month contract covering AMHP secondment Looking for candidate to start as soon as possible. You will need to be a car driver as you will be required to attend visits across in the Borough. A flexible approach is required, some home working and office work as and when required.

• Reviewing
• Duty Rota Assessment Clinic
• Office based
• Care Co-Ordination
• Attend MDT's and ward round

To apply for this role, you will need to hold -

• A minimum of 6 months experience of working in the UK as a registered social worker
• A relevant Social Work Qualification
• Registration with Social Care Wales / Social Work England
• Enhanced DBS, registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
• Driving license with access to a car

• 20 + years’ experience in the social work market
• A local specialist agency based in Cardiff, with roles across South Wales, West Wales and South West England
