Mersey Care NHS Foundation Trust is recruiting a Support Worker to join the Hospital Mental Health Liaison Team across Aintree and partner hospitals. You will contribute to mental health assessments and provide direct care and observation under supervision in the acute hospital setting.

You will work across ages 16+ to older adults, in A&E and ward environments, signposting to community services, and supporting discharge planning.

Responsabilidades

• To carry out assigned duties, involving direct care and observation of service users under the guidance and indirect supervision of qualified staff.
• To report to the qualified staff any observations made whilst providing care to service users / productivity and leisure, providing details of mental state and risk factors.
• SW will have the opportunity to work across all ages from 16+ to older age and across both A+E and ward environments.
• Work with service users on an individual and group basis as outlined in the care plan and as directed by the appropriate named lead professional.
