Job summary

Mersey Care NHS Foundation Trust is recruiting a Band 6 Senior Nurse Practitioner for the Warrington Recovery Team (Adult Community Hub). The role involves assessments for newly referred service users, MDT collaboration, and providing NICE-guided interventions to support access to appropriate care.

Qualifications

• RMN with post-registration development and CPD evidence.
• Experience leading and supervising staff within mental health services.
• Experience of managing cases & caseloads within a MDT setting.

Responsibilities

• Offer assessments of need for newly referred service users.
• Work as part of an MDT to provide care, treatment and support for those with complex needs.
• Lead triage of referrals and signpost to most appropriate interventions.
