Pennine Care NHS Foundation Trust in Stockport is seeking a Senior Mental Health Liaison Practitioner for a 12-month secondment to work within a multi-disciplinary team, responding to referrals from the Emergency Department and medical wards in the Acute trust.

You will assess, risk manage, signpost and refer, provide information to police and ambulance, and develop interfaces with other mental health services. The role includes 24/7 shifts and cross-borough collaboration.

Qualifications

• Must be a registered mental health professional with experience in assessing and supporting individuals of any age.
• Experience in hospital-based mental health services is preferred.
• Ability to assess risk, plan, and signpost or refer to appropriate services.

Responsabilités

• Assess patients' mental health needs and refer to appropriate services.
• Assess and manage risks and liaise with families and agencies.
• Ensure safe discharge to GP where appropriate.
• Prioritise referrals and manage workload.
• Provide evidence-based assessments and brief interventions.
• Maintain accurate records and information systems.
• Contribute to team development and meetings.
• Support student placements and learning outcomes.
• Ensure adherence to policies and participate in audits.
• Support performance monitoring and advocate for service funding.
