Job summary

Pennine Care NHS Foundation Trust is recruiting a senior mental health liaison practitioner for Oldham. You will assess referrals from A&E, hospital wards and police, plan care and support safe onward pathways.

The role focuses on relapse prevention, early discharge and improving quality of life for service users in acute and ward settings. You will join an experienced multidisciplinary team, providing high-quality assessments, training and support to hospital staff while maintaining patient

Qualifications

• Comprehensive mental health assessment and risk assessment for service users.
• Clinical assessments and interventions for relapse prevention, early discharge and QoL.
• Ability to work within a multidisciplinary Mental Health Liaison team.

Responsibilities

• Work within the Mental Health Liaison team to assess referrals from A&E, wards or S136.
• Carry out mental health assessments for individuals attending the Emergency Department.
• Provide support to hospital wards, health staff and police/ambulance colleagues to guide pathways.
