Job summary

Pennine Care NHS Foundation Trust is seeking a Senior Mental Health Liaison Practitioner to join the Bury Liaison Mental Health Team. You will undertake comprehensive assessments, plan care, and coordinate referrals for patients presenting to A&E, hospital wards, or under Section 136.

Working within a multidisciplinary team, you will provide risk assessments, support discharge planning, and offer training to ward staff and emergency services to improve pathways and patient safety.

Qualifications

• Professional registration in health or social care with up-to-date accreditation.
• Additional training relevant to the role.
• Experience in risk assessment.
• Knowledge of national legislation (MHA and MCA) and risk assessment practices.
• Knowledge of liaison roles with A&E and medical wards.

Responsibilities

• Work in a multidisciplinary Mental Health Liaison team to assess referrals from A&E, wards and S136.
• Perform comprehensive mental health and risk assessments and plan care for admission or discharge.
• Provide information and support to police, ambulance staff and ward staff; coordinate pathways.
• Support and train acute hospital staff on recognition and management of mental health conditions.
