Job summary

Pennine Care NHS Foundation Trust is seeking a Senior Liaison Mental Health Practitioner to join the Bury Liaison Mental Health Team. The role involves carrying out comprehensive mental health assessments and risk assessments for patients attending A&E, hospital wards, or Place of Safety.

You will work within a multidisciplinary team to support relapse prevention, early discharge, and coordinated care planning.

Qualifications

• Must be a Qualified Mental Health Professional.
• Experience in high-pressure clinical settings is desirable.

Responsibilities

• Carry out mental health assessments for patients presenting to the Emergency Department, hospital wards or Place of Safety.
• Provide comprehensive risk assessments and management plans.
• Work within a multi-disciplinary Mental Health Liaison team to support relapse prevention and early discharge.
• Support wards with patients having complex physical and mental health needs including older adults and younger people.
• Deliver training and guidance to acute hospital staff on recognizing mental health conditions and appropriate onward referrals.
• Liaise with police and ambulance staff to inform pathway decisions.
• Coordinate assessments and care planning during patients brought to Place of Safety under Section 136.
• Act as shift co-ordinator in the absence of the clinical lead and triage referrals.
• Contribute to service planning, evaluation and development, and comply with Trust policies.
