Job summary

Pennine Care NHS Foundation Trust is seeking a Community Support Worker to join the Early Intervention Team. The role focuses on delivering socially inclusive mental health services to individuals experiencing first episode psychosis, with duties ranging from health referrals to supporting personal goals and independence.

You will assess needs, coordinate with practitioners, and help service users access education, employment and housing services, while maintaining documentation and engaging

Qualifications

• NVQ 3 or equivalent experience.
• Commitment to undertake continuing personal development.
• Evidence of recent further development/training.

Responsibilities

• Highlight health concerns to Care Coordinator, GP and Consultant Psychiatrist.
• Refer clients for appropriate interventions and health advice.
• Assist in planning and evaluating individual care plans.
• Support service users in engaging with benefits, employment, housing and substance misuse services.
• Promote independent living and provide practical help and emotional support.
• Provide health promotion information and support.
• Monitor service users’ mental state and escalate concerns as needed.
• Report progress and early signs of relapse to the team.
• Maintain accurate documentation per Trust policies.
