Buckinghamshire Mind is seeking 2 x Community Connectors to join our team across Berkshire, Windsor, Ascot and Maidenhead, with community-based and remote elements. You will support people to access social care, housing, debt and employment services, promoting wellbeing and recovery.

The role requires a degree or equivalent experience in support work or community engagement, excellent communication, empathy, and a willingness to travel between locations using a vehicle. DBS checks apply.

Qualifications

• Qualified: relevant degree/diploma or equivalent experience.
• Experience in support work or community engagement.
• Excellent communication and collaboration skills.
• Ability to work in a multi-disciplinary team.
• Willingness to travel; driving licence preferred.

Responsabilités

• Work as part of a multi-disciplinary team in primary care.
• Collaborate on assessments and goal setting with the Mental Health Practitioner.
• Provide self-management tools and support personal plans to promote independence.
• Build relationships with agencies, GPs and professionals for coordinated care.
• Help patients access services like Safe Havens, CMHT and well-being programs.
