Buckinghamshire Mind is seeking 2 Community Connectors to join our team, covering the Berkshire area with a community/remote working arrangement.

You will support people to access a wide range of community services, coordinate with social care, housing, family, debt and employment resources, and promote wellbeing. This role requires travel within Windsor, Ascot and Maidenhead, 37 hours per week, permanent, £25,250 per year.

Qualifications

• Qualified with relevant degree, diploma, or equivalent experience.
• Experience in support work or community engagement.
• Full driving license preferred; access to a vehicle.
• Ability to work in a multi-disciplinary team.

Responsabilités

• Support patients to access community services and resources.
• Attend initial client assessments and assist with risk assessments.
• Provide self-management tools and support plans.
• Build relationships with agencies, GPs and healthcare professionals.
• Ensure continuity of care and travel between locations.
