Gateshead Health NHS Foundation Trust seeks a highly motivated Team Manager to lead the Older Persons Specialist Memory Hub in Gateshead, improving diagnostic times and post-diagnostic support for dementia patients. You will oversee a well-established Memory Hub, driving high-quality, safe, compassionate care for older adults with dementia.

Formación

• Registered on the NMC register, Mental Health.
• Mentor qualification / ENB 998 or equivalent/ Completed Mentorship Course.
• Mentorship/Teaching & assessing qualification.
• Leadership Qualification (Desirable)

Responsabilidades

• Provide management to the team with accountability and responsibility for the safe delivery of all aspects of patient care.
• Support the Clinical Operational Manager in continuous service improvements and the delivery of transformational projects within areas of accountability.
• Monitor the performance and quality of the service the team delivers and ensuring local and national targets are met.
• Ensure the experience of our patients and their families is understood within the teams.
• Ensure the highest standard of holistic safe and effective care is delivered through the use of evidence based professional practice, in line with agreed local and national standards.
• Responsible for the effective and efficient operational management of the Memory Hub.
• Support and continually developing staff to ensure they have the appropriate level of skill, knowledge and competence to meet service/speciality requirements
