Job summary

Tees Esk and Wear Valleys NHS Foundation Trust seeks a Clinical Specialist Nurse (Memory Services) to lead memory service work across Scarborough Whitby & Ryedale, driving the redesign of the memory pathway in collaboration with Primary Care, VCSE and other partners. The role supports diagnosis, capacity, and care pathways within the community setting.

Qualifications

• Registered RMN/RNLD with current NMC registration.
• Experience in memory assessment and dementia diagnosis.
• Experience in leading and supervising clinical teams.
• Willingness to travel across the locality with access to a vehicle.

Responsibilities

• Provide specialist advice, training, therapeutic support and consultation to Primary Care and VCSE with the Memory Assessment Service Leadership Team.
• Develop relationships and processes to enable safe, timely dementia diagnosis within Primary Care.
• Support partnership development and evaluate the new service redesign for wider adoption.
• Be able to become/act as a Non-Medical Prescriber within agreed timescales.
• Promote community awareness of dementia and inform service investment decisions.
