Job summary

Practice Plus Group is hiring a Medicines Optimisation Pharmacist to cover the South Yorkshire region, including HMP Doncaster, Lindholme, Moorland and Hatfield. You will work across sites, delivering medicines optimisation and structured reviews as part of a wider health package for prisoners.

You will lead a team of pharmacists and technicians, ensuring high clinical standards, collaboration with stakeholders, and a patient-centered approach across all locations.

Qualifications

  • GPhC registered Pharmacist and Non-Medical Prescriber.

Responsibilities

  • Provide expert clinical advice on medicines and polypharmacy to ensure safe, modern medicines management and medicines optimisation in prisons.
  • Deliver the national company strategy around medicines optimisation and Structured Medication Reviews as part of healthcare package for the prison population.
  • Inspire leadership within the team of pharmacists and pharmacy technicians and support development to ensure high standards of clinical expertise locally at each site.
  • Promote good practice in prescribing and optimisation decisions while keeping patient engagement central to medicines optimisation.
  • Share information on medicines optimisation and evidence-based practices to maintain high standards and sound clinical decisions.

We have an exciting opportunity for a Medicines Optimisation Pharmacist to join our Pharmacy team covering the South Yorkshire Region.

This role covers HMP Doncaster, HMP Lindholme, HMP Moorland and HMP Hatfield which are all located in South Yorkshire and you will be required to work across all sites.

Hours

37.5 hours per week, Monday to Friday, core business hours.

Salary

This position is full time, you’ll receive an annual salary of £51,643 to £57,955per annum - depending on experience.

What you'll be doing

As a Medicines Optimisation Pharmacist with Practice Plus Group you will;

  • Provide expert clinical advice on medicines and poly-pharmacy in line with expected standards, ensuring that safe, modern, medicines management and medicines optimisation service are provided to patients.
  • Deliver the national company strategy around medicines optimisation and Structured Medication Reviews which will be part of a comprehensive healthcare package based on the prison population needs.
  • Inspire positive and aspirational leadership to the team of pharmacists and pharmacy technicians and support the development of both to ensure high standards of clinical expertise are available locally at each site.
  • Promote good practice in all prescribing and optimisation decisions while ensuring patient engagement remains as a focus of the medicines optimisation agenda.
  • Share information relating to medicines optimisation and good evidence-based practices to encourage high standards and sound clinical decision making processes.

Qualifications & Experience Required
  • GPhC registered Pharmacist
  • Non-Medical Prescriber
  • Available to newly qualified pharmacists and beyond
  • Management and leadership experience
  • Procurement and audit experience
  • Vocational Master’s Degree in Pharmacy including pre-registration training
  • Excellent communication skills
  • Experience developing effective working relationships with internal and external stakeholders
  • Non-judgemental and compassionate approach
  • Good IT skills and IT literacy
  • Ability to work within a busy environment
