Job summary

East Lancashire Hospitals NHS Trust seeks an experienced professional to coordinate Integrated Neighbourhood Team (INT) meetings and support the integration of health, social care and community services across Pennine Lancashire. You will act as the link between primary care, voluntary sector and community providers, promoting care closer to home.

• UK professional registration is required.
• Experience in community services and MDT working is desirable.
• Leadership and project management qualifications are desirable.

Responsibilities

• Chair and document regular INT multi-disciplinary discussions and record outcomes.
• Liaise with health, social care and community providers to ensure high-quality MDT working.
• Coordinate INT meetings and develop risk profiling links to GSF meetings.
• Promote INT activities and support continuous improvement.
