Job summary

Cornerstone Practice and Healthcare is seeking a Care Coordinator to manage the Local Enhanced Services (LES), coordinate Multidisciplinary Team (MDT) meetings and End of Life Care support across four sites in Blackburn and Oswaldtwistle.

You will work with GPs, nursing staff and the Quality Manager to ensure patient cohorts are identified, reviewed and supported, delivering high‑quality care with a warm, patient‑centred approach.

Qualifications

• GCSEs including Maths and English or equivalent.
• NVQ Level 3 in Business Administration or equivalent.
• Demonstrable literacy and numeracy skills.
• Understanding of data protection legislation and patient confidentiality requirements.

Responsibilities

• Maintain LES specifications and contract alignment.
• Ensure timely claim submissions.
• Data verification for correct coding and payments.
• Coordinate MDT meetings and End of Life Care coordination.
• Use population health tools and EMIS Web to identify patient cohorts.
• Support nursing visits and monthly nursing clinics.
