Job summary

Locum Ready is recruiting a Practice Nurse to join a GP practice in Swaffham. The successful candidate will deliver chronic disease reviews, wound care, immunisations and routine clinics in a structured primary care setting.

You will contribute to high-quality patient care through clinical reviews, preventative health work and treatment room activity, with well-organised workflows and supportive nursing leadership for smooth daily clinic delivery.

Qualifications

• NMC-registered nurse with experience in primary care.
• Immunisation and cytology training desirable.
• Confident in chronic disease management and treatment room-work.
• Good patient communication and clinical reasoning.
• Familiarity with EMIS, SystmOne or Vision.
• Enhanced DBS.

Responsibilities

• Conduct chronic disease reviews for asthma, COPD, diabetes or hypertension.
• Perform wound care, dressings and treatment room duties.
• Carry out adult and children’s immunisations (if trained).
• Undertake smear tests (cytology-trained nurses).
• Complete NHS Health Checks and lifestyle assessments.
• Manage results, recalls and routine follow-up tasks.
• Document clearly in the clinical system.
