Job summary

Lancashire County Council is seeking a Senior Social Worker to join the Hospital Discharge team, supporting safe and timely discharges from hospital in the Adult Social Care service.

You will work with health colleagues, families and partners to promote recovery and independence, ensuring a positive discharge experience and person-centred care.

Qualifications

• Social Work England registration required.

• DBS Disclosure and Barring Service required if successful.

The role

Joining our Hospital Discharge team as a Senior Social Worker in this role y ou will support safe, timely and person-centred discharges from hospital, working closely with health colleagues, families and wider partners. You will help people move from hospital to the most appropriate setting, promoting recovery, independence and a positive discharge experience.

Social Work applicants must be registered with Social Work England (SWE) to be appointed as a Social Worker or have applied to register with SWE.

Local Government Reorganisation

The Government has confirmed that local government across Lancashire will be reorganised, with the current 15 councils being replaced by four new unitary councils covering North, South, East and West Lancashire from April 2028.

If you are successful in your application, you will initially join Lancashire County Council. However, as part of the planned transition, your employment will transfer to one of the new unitary councils from April 2028, subject to the implementation of the reorganisation and relevant employment legislation.
