Job summary

Age UK Nottingham and Nottinghamshire is seeking an Area Coordinator for the Living Well at Home Service in Meadows, Nottinghamshire. You will be the first point of contact for new clients, assess needs and create personalised support plans while supervising a small team of Living Well Assistants.

This permanent, 20-hour-per-week role offers flexible working and a pension. Closing date is 26 October at 9am.

Qualifications

• Excellent communication and organisational skills.
• Strong IT competency for recording and managing client information.
• Understanding of person-centred support for older and vulnerable people.
• Experience supervising staff, preferably in domiciliary care, home support, social care or community services.

Responsibilities

• Assess clients to understand their needs and create tailored support plans
• Communicate effectively with clients, families and the wider team
• Supervise and support a team of Living Well Assistants
• Maintain strong organisational systems and IT records
• Coordinate service delivery across the local community
