Job summary

Care Santé Limited in Nottingham is seeking a confident Care Coordinator to support our Nottingham home care service. You will plan care calls, roster workers, and liaise with clients and the Local Authority to grow services.

This role requires driving, previous health and social care experience (level 3 or equivalent), and comfort working across our Long Eaton and Head Office in Alfreton locations; on-call duties are included and paid on top of salary.

Qualifications

• Experience in health and social care settings is required.
• Must have or be working towards a Level 3 qualification or equivalent.
• Ability to plan rosters and manage care calls effectively.

Responsibilities

• Plan care calls and produce weekly care worker rosters using UniqueIQ.
• Coordinate assessments, reviews, and service enquiries with authorities.
• Supervise frontline staff, recruit and support care workers, and handle on-call duties.
• Maintain training records and respond to client queries across locations.
• Escort and support carers with tools and medication checks when needed.

Job description

We're recruiting for a new Care Coordinator to support our Nottingham home care service.

What's on offer:

• Working 37.5 hours a week- Monday to Friday 9am to 5pm
• Participating in the on-call rota is a requirement for the role

This role will be primarily based at our Long Eaton office, with regular working required at our Head Office in Alfreton. The successful candidate should therefore be comfortable working across both locations as required.

As a Care Coordinator your duties will include:

• Planning care calls and producing efficient daily care worker rosters using our UniqueIQ system. The advance roster must be created in advance of a minimum of one week, ideally two weeks in advance.
• Facilitating customer care assessments and reviews.
• Close cooperation with The Local Authority and potential customers to gain new contracts, and all tasks relevant to the enquiries.
• Supervisory responsibility for frontline staff including care worker supervisions, on-site spot checks and competency assessments
• Supporting recruitment by interviewing care worker applicants
• Crating new care staff users on the UniqueIQ
• Crating new customers users on the UniqueIQ
• Assisting carers with mobile phones. (set up, monitoring, support).
• Supporting with medication checks, transcribing and e-MAR chart audits
• Completing ECM system reconciliations (CM 2000 and IQ Timecard)
• Being a consistent point of contact for customer queries and issues
• Maintaining the training matrix and evidence within UniqueIQ
• Participating in the service's on-call rota (this is paid on top of salary, one in four weeks).
• Undertaking other office duties as required
