Job summary

Rebuild 4 Life Limited in Liverpool is seeking a compassionate Wake Night Support Worker to join the care team, delivering person-centred care for a 19-year-old female with complex needs and cerebral palsy in a home setting.

You will administer medication, assist with PEG feeding and mobility, document care accurately, accompany medical appointments, and help with light housekeeping while coordinating with the MDT to review personalised care plans.

Qualifications

• Experience in complex care is essential.
• Knowledge of medical terminology and care planning.

Responsibilities

• Assist clients with daily activities including personal hygiene, dressing, and mobility support.
• Administer medication in accordance with prescribed care plans and medical guidelines.
• Support clients with PEG feed preparation and nutritional needs, ensuring dietary requirements are met.
• Maintain accurate documentation of care provided and any changes in client condition.
• Support client with medical appointments and accompany them when necessary, including driving to appointments.
• Assist with light housekeeping tasks to maintain a clean and safe environment.
• Collaborate with healthcare professionals to develop and review personalised care plans.
• Respond promptly to emergencies or urgent needs following first aid protocols.
