Qualifications

• Experience supporting care leavers or young people transitioning to adulthood
• Full UK driving licence and access to a vehicle insured for business use
• Enhanced DBS certificate
• Strong communication and advocacy skills
• Ability to work collaboratively with partner agencies
• Excellent organisational and record-keeping skills

Responsibilities

• Build positive relationships with care leavers and provide ongoing guidance and practical support
• Develop and review pathway plans to support transition to adulthood
• Assist with accommodation, tenancy setup and home setup
• Advocate for young people and coordinate with partner agencies
• Promote health and well-being and access to services
• Provide financial guidance to help manage money
• Maintain accurate records and manage competing priorities
