Summary:

The Leaving Care Worker is a pivotal point of contact and coordination for young people aged 16 to 25 as they transition from local authority care to independent adulthood. Operating at the interface of children's and adult services, you will provide a flexible and reliable connection to young people, ensuring they move successfully through life toward becoming happy, healthy, and self-sufficient adults. This role requires a proactive approach to engaging "harder to reach" individuals and a high level of coordination to bring together multi-agency resources in line with the Council's Children in Care and Care Leavers Strategy.

Responsibilities:
  • Use specialized skills to engage with young people who choose not to work with services or are considered harder to reach, building connections to enhance their life chances.
  • Lead on the coordination and implementation of individual Pathway Plans for young people transitioning from care.
  • Identify and support young people experiencing elevated levels of risk, including poor mental health, anti-social behavior, or the risk of losing employment and housing.
  • Ensure all transitions are managed in a "safe and healthy manner," maintaining awareness of both children's and adult service frameworks.
Qualifications:
  • A relevant qualification in Social Care, Youth Work, or a related field (typically NVQ Level 3 or equivalent).
  • A high standard of general education, including English and Maths.
Essentails:
  • Proven experience of working directly with young people (ideally aged 16–25) in a social care, youth work, or community setting.
  • Experience in supporting vulnerable individuals through significant life transitions, particularly moving toward independent living.
  • Understanding of the legislative frameworks governing both Children's Services and Adult Social Care.
Please Note:

  • You should have right to work in U.K
