Job summary

Qualifications

• Qualified social worker with DipSW or equivalent.

• Experience with looked after children and care leavers.
• Excellent partnership working and cross‑team collaboration.

Responsibilities

• Work with the Pathway Leaving Care team to support looked after children and care leavers.
• Collaborate with partners and internal teams to improve outcomes.
• Maintain timely casework, plan and review progress.
• Report to Team Manager and adhere to DBS checks.
