Job summary

Bolton Council is hiring a Leaving Care Support Worker to assist young people aged 16-25 as they transition from care to independent living. You will provide flexible, person-centred support and work with families, carers and partner agencies to assess needs and deliver outcomes.

You will help develop Pathway Plans, support with housing and education, and coordinate services while maintaining records and safeguarding responsibilities. Driving licence and transport access are required.

Qualifications

• Experience working with young people.

• Ability to manage a caseload and prioritise tasks.
• Commitment to safeguarding and equality.
• Knowledge of childcare legislation, policies and procedures.

Responsibilities

• Support young people aged 16-25 transitioning from care into independent adulthood.
• Assist with development, implementation and review of Pathway Plans.
• Provide guidance on accommodation, independent living skills, education, employment and training.
• Support homeless or at-risk young people to access services.
• Manage caseloads and coordinate appropriate support with professionals.
• Attend pathway planning meetings and case conferences.
• Maintain accurate records and uphold confidentiality and safeguarding.
