Job summary

Belmont Recruitment is seeking an experienced Leaving Care Worker in Beverley on a temporary basis for three months, 37 hours per week, Monday to Friday. You will support care leavers aged 16 to 25 as they transition to adulthood, developing pathway plans and practical skills for independent living.

Responsibilities include building relationships, coordinating with partner agencies, and guiding access to housing, finances, health services and tenancy support.

Qualifications

• Previous experience with care leavers or youths transitioning to adulthood.
• Understanding pathway planning and challenges faced by care leavers.
• Full UK driving licence with access to a vehicle.
• Enhanced DBS certificate.
• Strong communication and advocacy skills.
• Ability to work with partner agencies and professionals.
• Excellent organisational skills and accurate records management.

Responsibilities

• Build positive relationships and provide ongoing advice, guidance and practical support.
• Develop and review pathway plans to support transition to adulthood and independent living.
• Support access to suitable accommodation and tenancy skills.
• Assist with setting up and maintaining a home.
• Advocate for young people and coordinate with partner agencies to align support.
• Promote health, wellbeing and access to relevant services.
• Provide financial guidance to help manage money and independence.
