• Build personalised pathway plans for care leavers to set goals and provide tools to achieve them.
• Coordinate with external agencies (education, housing, employment services) to create a support network for each care leaver.
• Maintain comprehensive records and reports to track progress and adjustments to pathway plans.
• Advocate for care leavers in various settings, ensuring their voices are heard and needs are met.
