Job summary

Service Care Solutions Ltd is recruiting for Leaving Care Personal Advisors in Birmingham. You will work with care leavers aged 18–25, supporting them towards independent adulthood and coordinating plans with professionals.

Ideally you have experience in leaving care, caseload management, safeguarding, and multi‑agency collaboration. You will help with accommodation, education, employment, benefits and other services while maintaining detailed records.

Qualifications

• Experience working with care leavers or young people 18+
• Ability to manage a caseload and complete assessments, plans and reviews
• Experience in multi‑agency and partnership working
• Knowledge of safeguarding policies and relevant legislation
• Support young people with accommodation, education, employment, benefits and independent living

Responsibilities

• Work with eligible, former relevant and qualifying care leavers aged 18–25.
• Complete and review Pathway Plans with young people and professionals.
• Help young people prepare for independent living and adulthood.
• Support access to housing, education, employment, training, benefits and services.
• Build positive relationships and involve young people in decisions.
• Assess needs and coordinate appropriate support packages.
• Coordinate with social workers, carers, families and partner agencies.
• Support continuing care needs and positive transitions into adulthood.
• Maintain accurate case records, assessments and plans.
• Participate in duty and referral work as required.
• Advocate for entitlements and available services.
• Contribute to service development and multi‑agency working.

Location: Birmingham Service: Leaving Care / 18+ Services Multiple opportunities available We are currently recruiting for several Leaving Care Personal Advisor positions to join Birmingham’s 18+ Services, supporting care leavers aged 18–25 as they take the next steps towards independent adulthood. This is a fantastic opportunity for an experienced Personal Advisor, Leaving Care Worker, Care Leaver Advisor or Social Care professional who is passionate about helping young people build brighter, more independent futures.

What will you be doing?

• Working with eligible, relevant, former relevant and qualifying care leavers aged 18–25.


• Completing and reviewing Pathway Plans alongside young people and relevant professionals.


• Supporting young people to prepare for independent living and adulthood.


• Helping care leavers access appropriate accommodation, education, employment, training, benefits and other services.


• Building positive, trusting relationships and ensuring young people remain actively involved in decisions about their future.


• Assessing individual needs and coordinating appropriate support packages.


• Working alongside social workers, carers, families and partner agencies.


• Supporting young people with continuing care needs and helping them achieve a positive transition into adulthood.


• Maintaining accurate and up-to-date case records, assessments and plans.


• Participating in duty and referral work when required.


• Advocating for young people's entitlements and ensuring they receive the services and support available to them.


• Contributing to wider service development and multi-agency working.

What are we looking for?

You will ideally have experience of:

• Working with care leavers or young people aged 18+.


• Managing a caseload and completing assessments, plans and reviews.


• Supporting young people towards independence.

• Safeguarding and working within relevant legislation, policies and procedures.
