Job summary

Sheffield Inclusion Centre is seeking a dedicated Learning Mentor to support children and young people through school transitions and between services. You will collaborate with schools to identify needs, provide targeted mentoring, and help ensure smooth information flow across settings.

This role emphasizes safeguarding, equality, and inclusive practice within a supportive team. The post focuses on developing personalised action plans, monitoring progress, and delivering mentoring strategies

Responsibilities

• Assist children and young people to make a successful transfer between schools/services and transition at key stages in their learning.
• Lead in identifying support needs of individual children at transfer points between schools or services and during transitions between education phases.
• Ensure speedy and effective transfer of information within and across schools or services and settings.
• Take a lead role in maintaining continuity of support throughout transfer and transition, including summer schools.
• Contribute to comprehensive assessment of children entering schools or services and review their progress and achievements.
• Assist in identifying barriers to learning for individual children and provide strategies to overcome them.
