Responsabilidades

• Undertake safeguarding enquiries and risk management.
• Carry out Mental Capacity Assessments and Best Interest decisions.

• Collaborate with adults, families, carers, health professionals and providers.

The role

As an Adult Social Worker, you will support adults across a range of community settings, including:

• Private homes
• Supported-living services
• Residential homes
• Nursing homes

You will manage a varied caseload and undertake meaningful, statutory Adult Social Care work, including:

• Safeguarding enquiries and risk management
• Mental Capacity Assessments and Best Interest decisions
• Court of Protection work
• Assessing, implementing and reviewing care and support packages
• Managing referrals for supported living, residential and nursing placements
• Working closely with adults, families, carers, health professionals, providers and partner agencies
• Delivering strengths-based assessments, care planning and case management focused on independence and positive outcomes

What were looking for

• A recognised Social Work qualification
• Current Social Work England registration
• Previous experience within Adult Social Care
• Strong working knowledge of adult safeguarding, the Mental Capacity Act and Best Interest processes
• Confident assessment, care planning and case-management skills
• A person-centred approach and commitment to helping adults achieve the outcomes that matter to them
• The ability to manage competing priorities while maintaining high-quality case recording and practice
