Job summary

Leaders in Care are recruiting an Adult Social Worker for a council team in the Hounslow area. The role focuses on balancing manageable caseloads with professional development and work-life balance, while offering hybrid working and home-based support.

You will work across community settings, including homes and care facilities, delivering Court of Protection work, safeguarding inquiries, Best Interest Assessments, and care package reviews.

Qualifications

• Social Work qualification required.
• Current registration with Social Work England required.
• Experience in adult social care and safeguarding knowledge essential.
• Strong assessment, care planning and case management skills.

Responsibilities

The role

You’ll support adults across a range of community settings, including people’s homes, supported living services, residential homes and nursing homes. Your responsibilities will include Court of Protection work, safeguarding enquiries, Best Interest Assessments, and implementing and reviewing care packages. You’ll also manage referrals for supported living, residential and nursing placements.
