Job summary

Leaders in Care is looking for an experienced Adult Community Social Worker in Wales (Caerphilly). The role offers flexible/hybrid working with up to 3 days from home and a supportive team.

You will conduct strengths-based assessments and reviews under the Care Act 2014, manage a community caseload, safeguard adults at risk, and coordinate care with families, carers and health professionals to enable timely discharges.

Qualifications

• Qualified Social Worker registered with Social Work England (SWE).
• Experience with older persons' mental health or related services.
• Ability to manage a community-based caseload and conduct Care Act assessments.

Responsibilities

• Conduct strengths-based assessments and reviews under the Care Act 2014.
• Support independence and create care and support plans.
• Collaborate with families, carers, health professionals and providers.

The Role

You will manage a community-based caseload, complete Care Act assessments and reviews, undertake mental capacity and best-interest assessments where required, and work closely with families, carers, health professionals, providers, and voluntary-sector partners. You will also play an important role in safeguarding adults at risk and supporting timely hospital discharges where appropriate.
