Job summary

Leaders in Care is assisting a council to recruit an Adult Social Worker to join a supportive team with manageable caseloads and strong work-life balance. The role covers community-based work across homes, supported living, and residential settings, with a focus on person-centred care.

You'll conduct assessments, safeguarding, Best Interest processes, and referrals for placements, while benefiting from hybrid/home working and a dedicated consultant support throughout your placement.

Qualifications

• Social Work qualification and current registration with Social Work England.
• Experience in adult social care with safeguarding, mental capacity and Best Interest knowledge.
• Strong assessment, care planning and case management skills; person-centred approach.

Responsibilities

• Support adults across community settings including people’s homes, supported living services, residential homes and nursing homes.
• Manage referrals for supported living, residential and nursing placements.
• Collaborate on safeguarding, Best Interest assessments and Court of Protection work; implement and review care packages.

The role

You’ll support adults across a range of community settings, including people’s homes, supported living services, residential homes and nursing homes. Your responsibilities will include Court of Protection work, safeguarding enquiries, Best Interest Assessments, and implementing and reviewing care packages. You’ll also manage referrals for supported living, residential and nursing placements.
