Job summary

Pennine Care NHS Foundation Trust is seeking a Learning Disability/Mental Health Liaison Clinician to join the Learning Disability Care Hub. You will undertake specialist assessments, develop care plans, and provide expert liaison across adult LD and complex mental health presentations in a range of settings including homes.

You will work with families, carers, mental health services, social care and acute teams to prevent crises and promote independent living.

Qualifications

• Experience as a community nurse.
• Experience supervising others.
• Knowledge of learning disability and mental health care services.

Responsibilities

• Undertake specialist assessments of mental health and learning disability and formulate care plans.
• Deliver person-centred interventions and specialist guidance to professionals and carers.
• Coordinate safeguarding, risk management and prompt crisis intervention.
• Support hospital discharge planning and community reintegration.
• Collaborate with primary and secondary care, social care, families and partner agencies.
