Job summary

York Teaching Hospital NHS Foundation Trust is seeking a Learning Disability Liaison Nurse to provide a specialist service across York and Scarborough acute hospitals. You will manage a defined caseload, assess needs, coordinate complex care planning and ensure reasonable adjustments are implemented to improve patient experience and outcomes.

The role involves delivering LD awareness training, working with multidisciplinary teams, and contributing to service development.

Responsibilities

Manage a defined caseload of patients with learning disabilities accessing acute healthcare services.
Assess, plan, implement and evaluate specialist care and treatment programmes.
Coordinate complex patient pathways, including admission, treatment and discharge planning.
Provide specialist advice regarding reasonable adjustments, consent, best interests decisions and the Mental Capacity Act.
Act as a specialist point of contact for patients, carers and professionals.
Support patients and carers to understand healthcare information through accessible communication.
Identify and address barriers to accessing acute healthcare services.
Deliver learning disability awareness training and education programmes.
