Qualifications

• Registered RNLD with current NMC registration.
• Educated to degree level.
• Knowledge of NHS Continuing Healthcare and Nursing Funded Care.
• Awareness of Mental Capacity Act Assessments.
• Awareness of financial management, brokerage and invoicing.

Responsibilities

• Undertake NHS Continuing Healthcare assessments for individuals with learning disabilities and complex health needs.
• Develop, implement and coordinate person-centred care plans with individuals, families, carers and multidisciplinary teams.
• Advocate for individuals, ensuring their voice and preferences are central to decisions.
• Coordinate, monitor and review complex care packages with services to achieve positive outcomes.
• Lead multidisciplinary meetings, case conferences and reviews with partners.
• Support safeguarding, risk assessment and risk management planning.
